What is liposuction for gynecomastia?
Liposuction for gynecomastia removes the fatty layer of the male chest through two or three small openings, leaving an outline that follows the muscle underneath. Because only fat can be suctioned, this operation suits a chest whose fullness is soft rather than firm. Gynecomastia Liposuction is therefore a contouring procedure, and it is not a treatment for glandular tissue.
Many men arrive believing that one operation covers every kind of chest fullness. Sadly the tissue does not behave that way, so what the chest actually contains has to be established by examination first.
Why must the cause of gynecomastia be looked for first?
True gynecomastia is a diagnosis before it is an operation, so the reason for the glandular growth is investigated before any surgery is planned. Several causes are common and a good number are reversible. Therefore a careful history, a full examination and, where indicated, blood tests come ahead of any theatre date.
- Normal puberty, which produces a tender disc behind the nipple in many teenage boys and often settles on its own within a year or two.
- Medicines and substances, including anabolic steroids, some blood pressure medicines, some antipsychotics and cannabis.
- Liver disease, thyroid disease and kidney disease, each of which changes the way hormones are handled by the body.
- Rarely, testicular or adrenal tumours, which is why examination of the testes forms part of the assessment.
Above all, a firm one sided lump in an older man needs proper assessment, because male breast cancer exists even though it is uncommon. Surgery is considered once causes have been excluded or treated and the chest has stayed stable for some months.
How is glandular gynecomastia different from fatty pseudo gynecomastia?
Glandular gynecomastia is firm breast tissue sitting directly behind the nipple, whereas fatty pseudo gynecomastia is soft fat spread across the chest wall. Consequently the treatment differs, and confusing the two is the commonest reason a man is unhappy afterwards.
| Feature | Glandular gynecomastia | Fatty pseudo gynecomastia |
|---|---|---|
| How it feels | Firm rubbery disc under the areola | Soft and evenly spread |
| Tenderness | Often tender, especially early on | Usually not tender |
| Change with weight loss | Very little | Frequently improves |
| Usual treatment | Excision of the disc, generally with suction | Liposuction alone in suitable men |
Which chest changes need assessment before any cosmetic plan?
Certain changes need prompt medical assessment rather than a cosmetic consultation, and they should never be worked around. Please arrange an appointment quickly where any of the following appear.
- A new lump, a growing lump, or a lump that feels hard or fixed to skin or chest wall.
- Discharge from the nipple, particularly where it is bloody or comes from one side only.
- A nipple that has newly turned inwards or become retracted.
- Dimpling, puckering or an orange peel texture of the skin above the area.
- Persistent pain on one side, or a quickly widening difference between the two sides.
These can be signs of breast cancer in men, so they are assessed with examination and imaging such as ultrasound or mammography. Cosmetic surgery is never the first step here. Once a worrying cause has been ruled out, contouring can be discussed calmly.
Can suction remove the firm disc under the nipple?
No, suction does not remove firm glandular tissue, and this single fact decides whether the operation will please you. A cannula travels through fat easily, while dense fibrous gland resists it and stays put. Consequently the disc under the areola remains, and it can even look more obvious once the surrounding fat has been taken away.
Men who are promised otherwise are frequently disappointed. Some describe a chest that is flatter overall yet still peaked at the nipple, which is the classic signature of suctioning fat and leaving gland behind. Where gland is the real problem, therefore, the disc has to be removed directly through a small incision, and that different operation is explained on our gland excision page.
Who is a good candidate for suction alone?
Gynecomastia Liposuction on its own suits a man whose chest fullness is soft, evenly spread and free of a firm disc, and whose skin still has reasonable elasticity. Importantly, age matters less here than skin quality. Additionally, body weight should be steady, since later gain or loss will change the shape achieved.
- Soft fatty fullness with no clearly palpable glandular disc behind the areola.
- Skin that springs back when the chest is pinched and released.
- Stable weight for at least six months, with no ongoing anabolic steroid use.
- Health suitable for anaesthesia, with any liver, thyroid or kidney condition already managed.
By contrast, a man with a firm disc, very loose skin or an unexplained one sided lump is not served by suction alone. In those situations plain speaking at the consultation saves both money and disappointment later.
How is the procedure done?
The operation is usually carried out under general anaesthesia, or under sedation with local anaesthesia, and it takes roughly an hour for both sides. Fluid with local anaesthetic and adrenaline is infiltrated first to limit bleeding. After that, a fine cannula passes through openings hidden in the fold at the side of the chest, and fat is taken evenly across the whole area.
Importantly, blending at the edges matters as much as the volume removed. Meanwhile the shape is checked repeatedly with the man sitting upright, since a chest that looks flat lying down can look quite different standing. Most men go home the same day.
What is recovery like?
Most men return to desk work within three to five days and to the gym at about four to six weeks. Soreness feels rather like a hard chest workout and responds to simple painkillers, while bruising fades over two to three weeks.
Compression is the part men neglect most often. Wearing the vest as instructed, generally day and night for four weeks and then during the day only, helps the skin settle onto the new contour.
What results should I expect?
Expect a flatter chest outline, with the final shape visible at around three to six months once oedema has resolved. Early on the area can feel firm and slightly uneven, which is normal scar tissue settling. Gradually that firmness softens.
Minor irregularities and mild asymmetry are common, since the two sides are rarely identical to begin with. Nobody can therefore promise a particular outcome. However, most men find that shirts sit better and that self consciousness eases a great deal.
What are the risks?
Risks include bleeding, a collection of blood or fluid under the skin, infection, contour irregularity, altered nipple sensation and sometimes the need for a second procedure. No operation is risk free, and hearing the list plainly is part of proper consent. Rarely a haematoma has to be drained in theatre.
- Dips or ridges where fat has been removed unevenly.
- Residual glandular fullness where a disc was present but not addressed.
- Numbness around the nipple, usually temporary though occasionally lasting.
- Skin that fails to shrink, leaving looseness that needs a separate plan.
Smoking, uncontrolled diabetes and blood thinning medicines all raise complication rates, so these are reviewed well beforehand. Should loose skin dominate afterwards, our page on skin tightening after gynecomastia sets out what can and cannot be done.
What does it cost in India?
Cost in India varies with the volume removed, the anaesthesia used and whether gland removal is needed as well. Charges quoted in rupees normally cover theatre, anaesthesia, the compression vest and routine review, while investigations are itemised separately.
Nobody can give a sensible figure over the telephone. Instead you receive a clear written estimate after examination, so the inclusions are known before you decide anything.
What are the common myths?
The largest myth is that suction flattens every male chest whatever it contains. Fat burning creams, chest straps and hormone supplements sold online do nothing for glandular tissue and occasionally make matters worse.
- Exercise cannot remove glandular tissue, although training certainly improves the muscle beneath it.
- Recurrence is uncommon once gland has been removed and the underlying cause dealt with.
- Lean men develop glandular gynecomastia too, so this is not purely a problem of body weight.
