Where is the nipple placed on a masculinised chest?

Placement lands on the lower outer part of the pectoral muscle, just above and slightly outside its lower edge. Sitting upright during marking is essential, since a chest lying flat gives a misleading picture. Symmetry gets checked from the front, from each side and from above before anything is stitched down. Small differences between the two sides are usual, because ribs and muscles are rarely a perfect match.

Why is placement judged from the pectoral border?

Judgement comes from the pectoral border because that muscle, and not the old breast, defines the shape of a masculine chest. Landmarks that suited the previous contour send a nipple too high and too close to the midline once the volume has gone. Muscle bulk also changes over years of training, therefore a border based landmark stays sensible as your body changes. Ribs and the collarbone serve as cross checks, so nothing is left to the eye alone.

How is the nipple resized and secured?

Resizing begins by trimming the areola to an agreed diameter using a circular guide, after which the nipple itself may be shortened if it projects too far. Where a graft is being used, the trimmed disc is thinned with care, because a thick graft will not take reliably. Fine stitches then hold it in position while a bolster dressing presses it gently onto the bed beneath. Where the nipple stays attached, as in the keyhole approach, the same trimming is done without detaching anything.

How does a graft take, and what does healing look like?

A graft survives by absorbing fluid at first, then by growing new blood vessels from the tissue beneath across roughly five days. Dressings therefore stay untouched during that window, since movement is the commonest reason a graft fails. Appearance in the first week is often dark and crusted, which is normal rather than a sign of trouble. Settling takes two to three months, while the finished look keeps improving for about a year.

Will the colour change afterwards?

Colour almost always changes to some degree after grafting, and most often it becomes lighter than before. Pigment cells are disturbed when the disc is thinned, therefore some loss is expected rather than unusual. Patchiness is common early and frequently evens out during the first year. Deeper skin tones sometimes show the opposite pattern, with the graft edge healing darker than the chest around it.

Can pigment loss be corrected?

Medical tattooing can restore colour and definition once the graft has fully matured. Waiting at least a year is usual, because pigment keeps shifting during that period and early work may simply need repeating. Can this be done as a separate operation later?
